Transitional Care

Transitional Care Management involves the transitioning of a patient from an emergency department (ED) or inpatient (hospital, skilled-nursing facility, short-term acute rehab, etc.) stay from the facility back to their home. Wings of Hope Medical Services team of professionals successfully close the large gaps in care that exist for patients and their caregivers during critical transitions. 

Our team utilizes Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) who assist in ensuring that a recently discharged patient receive a phone-call within two business days post discharge to review the patient’s discharge summary and ensure that the patient fully understands the information contained within it. The nurse will also schedule a follow-up appointment with the patient and their PCP within 7 calendar days for those identified as “high-risk” patients, 7-14 days for those with “moderate risk” patients (depending upon the medical necessity of the patient). 

High-quality transitional care is especially important for older adults with multiple chronic conditions and complex therapeutic regimens, as well as for their family and caregivers. These patients typically receive care from many providers and move frequently within health care settings, therefore they are particularly vulnerable to breakdowns in care and thus have the greatest need for transitional care services.

In providing this service, our patients have a decreased risk for readmission due to social determinants of health, medication errors or failure to recognize the signs and symptoms of a recurrence initiated appropriate treatment and prevent hospital readmissions

Telehealth Visits

Telehealth is a proprietary software developed to bring high-quality care to the patient wherever they may be.
With a combination of video-conferencing and an experienced team of medical providers, Telehealth brings the doctor’s office to the patient. Whether this takes place in a patient’s home, group-home, or a skilled-nursing facility, the patient will be able to access our Urgent Care service.

A provider would then join the patient in a video call, evaluate patient, discuss current patient condition. Once the visit concludes, the physician can write a script, order mobile labs and mobile diagnostic based on the patient needs, and provide an individualized treatment plan.
