How Does End-of-Life Care Work?
A physician, facility, or family member contacts Wings of Hope Medical Services to initiate coordination of hospice care. We respond quickly, often within hours.
Assessment and Onboarding
A nurse or care coordinator meets with the family (in person or via telehealth) to assess symptoms, discuss goals, and answer any questions.
Personalized Care Plan
Hospice develops a detailed, individualized plan that addresses physical, emotional, and logistical needs, incorporating input from family members and the patient.
Ongoing Visits and 24/7 Access
The team visits regularly to adjust care, manage changes in condition, and ensure comfort. Families can call us at any time for any reason.
Who Is End-of-Life Care For?
End-of-life care is appropriate for patients of any age who:
- Have a life expectancy of six months or less (as determined by a physician)
- Wish to focus on comfort rather than aggressive treatments
- Are living with advanced cancer, heart failure, ALS, dementia, kidney disease, or other terminal conditions
